What You Need to Know About Transplant

Members of the Transplant Team:

Your transplant team will work with you and your family every step of the transplant process to ensure your safety and well being throughout your care here at UMGCC. Members of the team include the following:

Physicians, fellows, and nurse practitioners
Nurses
Transplant nurse coordinators
Social work
Nutrition
Dentist and dental hygienist
Psychology and psychiatry
Physical therapy
Lab specialists
Financial coordinators and managed care specialists

Relationship-Based Care at the University of Maryland:

This care model was developed to:

Increase patient and family involvement in health care
Encourage continuity of care by providing a Primary Coordinating Nurse (PCN) to every patient. He or she is responsible for ensuring the coordination of your care while you are an inpatient.
